Course overview

This course explores the dynamic relationship between Community Action Agencies and the healthcare sector, focusing on collaborative strategies that address social determinants of health. Learners will be taken on a progression from learning the basics of social determinants of heath and health-related social needs through the role CAAs have in addressing these needs to the process of contracting with the healthcare sector to improve health outcomes in their communities. Through case studies and practical examples, the course highlights the critical role of community-based organizations in enhancing health outcomes and the importance of cross-sector partnerships in fostering sustainable well-being.
